Plate Replacement Options

You have several options to request a replacement license plate:

  1. By Mail Complete the attached Form HSMV 83146 and return it along with:

  • A copy of your valid photo identification

  • A check for $42.60, payable to Dariel Fernandez, Miami-Dade County Tax

In Person Visit the Miami-Dade County Tax Collector’s Office

At a Local Auto Tag Agency, you may also visit your nearest auto tag agency. Please note that additional fees may apply.
